Mrs. Spoon is a 48-year-old woman who works part-time as a kindergarten assistant and comes to the physiotherapy clinic with a history of Pain, numbness, and tingling in her right hand (particularly in the lateral three fingers) over the last 6 weeks, especially in night.(Insidious onset).
She recently started to have difficulty in using right hand for gripping and it is starting to affect the work as a kindergarten assistant. (She is right-hand dominant).
She feels the pain is getting worse because it is now extending up the forearm that wakes her up atleast once in the night. The pain and numbness is also making her weak in the functional activity like gripping, opening jars and working activities like packing up play equipment at kindergarten. At the end of the day she feels very painful. It has also affected her tennis which is her favorite leisure time sport activity. There is some relief while she changes position of her wrist and hand. Aspirin medication has helped a little in pain relief (started aspirin 2 weeks ago on advice of GP).
She was diagnosed with non-insulin-dependent diabetes 5 years ago, currently well controlled with diet and exercises.

Physical examination
Current pain on VAS is 2 and severe in the last week on VAS is 8.
No abnormality detected, no wasting of right thenar eminence but a slight reduction to light touch sensation on the palmar surface of the right thumb and 1st and 2nd finger noted. Most of wrist movements increases pain towards end range especially wrist extension is painful beyond 50°.
Grip strength on Jamar dynamometer: Right = 27 kg with pain on VAS is 3, Left = 35 kg). .Follow the guidelines below to write the assignment

Mr. Farhan 20 -year-old male Hockey player; experienced locking and snapping in his left knee during his regular practice. He was then referred to an orthopaedic doctor by his coach. He was doing well 4 months before he started to practice Hockey for the future intercollege athletic competition. During the practice session, he started to develop pain in a specific area of his left knee, which was similar to the current symptoms.

The patient reports pain on his medial compartment of knee and instability. Current symptoms increase on weight bearing through left lower extremity. He ignored the pain as it got relieved at rest. He practised additionally for 1 hour than the usual 1-hour practice session.
As the competition came closer, he started to practice vigorously. After starting vigorous practice, the pain in the left knee got provoked, and he was not able to play effectively. He rested for a few hours and then restarted the practice sessions. He ignored the pain as it could affect his participation in the competition if he consults a doctor for the pain.
Pain: – Site & side: Knee – left
Duration: 4 months
Aggravating Factor: Weight-bearing activities, Squatting and Twisting VAS: 5/10 On observation – No abnormality detected in the left knee and swelling in the medial compartment of left knee On palpation – Effusion in the medial compartment of left knee On examination – Active range of motion of the Left knee is within normal limits.
