1.0 Introduction

Write this:

Purpose: Set context, define the problem, and state what the report will do.

Industry snapshot: What product/service, where in Malaysia, basic supply-chain flow (supplier →production → distribution → customer).

Problem statement (one line): The key performance issue (e.g., “Frequent stockouts at DC cause late deliveries and lost sales.”).

Why now: Brief consequence (cost, service, time, quality).

Aim & objectives: What you will achieve using SWOT and Fishbone.

Scope: What is included/excluded (e.g., domestic distribution only; excludes export leg).

2.0 Literature Review

Purpose: Show you understand the tools and concepts you’ll apply.

Cover three short parts:

1. Supply chain performance basics (lead time, service level/OTIF, inventory turns).
2. SWOT for strategy selection (internal = S/W; external =O/T; strategy grid SO/WO/ST/WT).

3. Fishbone for root-cause (categories: Methods, Materials, Manpower, Machines, Measurement, Environment).

3.0 Methodology

Purpose: Explain how you did the work so it is repeatable.

Write this:

Design: “Qualitative case analysis with desktop research and review of literature.”

Steps (chronological):

1. Map basic supply chain.
2. Build SWOT (3–5 items per quadrant; prioritise top W/T).
3. Select one problem and conduct Fishbone.
4. Convert causes → SO/WO/ST/WT strategies.
5. Propose actions with KPIs.

Data sources: Company docs (if any), public reports; assumptions stated.

4.0 Analysis & Findings

Purpose: Present your results clearly: SWOT → Fishbone → Strategies → Actions.

Structure it in four sub-sections:

1. SWOT (table + short commentary)

o List 3–5 items per S/W/O/T; mark ★ top W/T you will address.
o Explain why the ★ item matters (impact on cost/time/service).

2. Problem Statement (1–2 lines)

o Clear, measurable: “Frequent stockouts at DC (ASKUs) → OTIF 86%; target ≥95%.”

3. Fishbone Diagram (one page figure + bullets)

o Summarise key causes under Methods/Materials/Manpower/Machines/Measurement/Environment.
o Identify critical few (2–3 causes with biggest impact).

4. Strategy & Action Plan (traceable to causes)

o Map causes → SO/WO/ST/WT strategies (at least 2per cell—keep them short).
o Convert to 3–5 actions with Owner | Timeline |KPI

5.0 Conclusion & Recommendations (≈200–300 words)

Purpose: Close the loop: what you found, what you will do, expected results.

Write this:

• One-paragraph summary of key problem and root causes.
• Top 3–4 recommendations (short, action-oriented, each with a KPI target).
• Expected outcomes (lead time ↓, OTIF ↑, cost-to-serve↓).
• Next steps (what to implement first; quick wins vs. medium-term).

• Table 1: SWOT (S/W/O/T with ★ priority).
